Clinical Trial Recruitment Companies: The Five Types and How to Choose

Registries, managed partners, community outreach, CROs and site systems solve different problems. Here is when each one fits.

The short answer

There is no single best clinical trial recruitment company. There are five different kinds of help, and they solve different problems. A common reason a study struggles to enroll is that the help it chose did not match the problem it had.

This guide describes the five kinds, names well-known examples of each, and gives you a way to decide. We are CliniContact, a recruitment partner. Our products cover four of these five, and we work alongside CROs on the fifth.

1. Registries, listings and trial finders

What they are. Public places where a study is listed and where people who are already looking for a trial can find it.

Examples.

  • ClinicalTrials.gov is the US government registry. Many clinical trials are required to register there, and it is where a lot of patients start their search.
  • ResearchMatch is a nonprofit program funded by the National Institutes of Health. It is a free recruitment and feasibility tool for researchers at participating institutions, and it connects them with volunteers who have signed up to hear about studies.
  • CenterWatch runs a clinical trials listing service that people can search by condition and location.
  • Power is a search site where patients browse trials by condition, location and treatment type.
  • Beacon (bridge-beacon.org) is CliniContact's participant site. People browse recruiting trials by condition, taken from ClinicalTrials.gov and checked daily, and build one research profile instead of filling in a new screener for every study.
  • Antidote matches patients to trials through its own search technology and a network of partner sites, and offers recruitment services to sponsors.

Best for. Every study. A complete, current registry record is the base layer, and if your institution takes part in ResearchMatch it costs nothing to use.

What to add. A listing reaches people who are already searching. To reach the much larger group who would qualify and have never thought about a trial, and to screen, follow up and schedule, pair it with active recruitment.

2. Managed recruitment partners

What they are. A team that plans the recruitment strategy for your study, builds the approved materials, runs the advertising, pre-screens the people who respond and hands your coordinators referrals that are ready to call.

3. Community and advocacy outreach

What it is. Building referral pathways through the organizations a patient community already trusts: advocacy groups, community clinics, provider networks and support organizations.

Best for. Rare diseases, conditions where people are not searching online, studies that need to reach communities paid advertising misses, and any study with representation targets to meet.

How we do it. Bridge is CliniContact's community outreach service. It maps the organizations for your indication and the area around your sites, reaches out to them by name, handles the replies, and shows the results in a live portal. Bridge is priced per indication, not per study: $1,200, $1,500 or $1,800 per indication a month, depending on how hard the community is to reach. How to plan it. Community relationships build over months and keep referring after the paid work ends. Run Bridge alongside digital recruitment, so referrals arrive from the first week while the community pathways grow.

4. Full-service CROs

What they are. Contract research organizations that run the whole trial and include recruitment as one part of it.

Examples.

  • IQVIA describes its patient recruitment as using real-world data and technology to find patients, target physician referral networks and improve engagement.
  • Fortrea offers rescue study services for delayed or underperforming trials, covering operational planning, recruitment support and site management.

Best for. Large, multi-country programs where recruitment has to be coordinated with site selection, monitoring and data management under one contract.

Where they stop. For a single-site or investigator-led study, a full-service contract is usually far more than the budget allows. A CRO can also bring in a specialist recruitment partner for a single study.

5. Site systems

What they are. The software a research team uses to manage participants and data once people have been found.

Examples.

  • REDCap is a secure web application for building and managing research surveys and databases, used widely at academic institutions.
  • CRIO provides eSource, eConsent and a site CTMS built for research sites.
  • RealTime-CTMS is a clinical trial management system for sites, covering scheduling, visit tracking and study finances.

Best for. Every site needs a system of record. These tools organize the people you already have.

How to choose

Ask these five questions in order.

  1. Is your registry record complete and current? If not, fix that first. It is free and everything else depends on it.
  2. Are the people you need already searching for a trial? If yes, listings and finders may be enough. If no, you need someone to go and find them.
  3. Do you have staff to write materials, run campaigns and call every lead? If not, you need a managed partner rather than a tool.
  4. Is the community hard to reach, or do you have representation targets? If yes, plan community outreach from the start, because it needs time.
  5. How much time is left? A study with a long runway suits an ongoing partnership. A study that is behind needs a short, fixed-term rescue with a specific outcome.

How much does clinical trial recruitment cost?

It depends on the kind of help. Registries and ResearchMatch are free. Full-service CROs quote per program. Managed partners often quote a price rather than publish one. Which recruitment companies pass ad spend through at cost?
